Please bring your completed intake forms, pertinent medical records, and reports, referral information if you have it, payment by cash or check. Please wear comfortable clothing, shorts, or pants that you can roll up over your knees.

No, if you would like to use the facility at other times, you can buy a day pass or become a member.

No, you do not need a referral to see a physical therapist in Massachusetts. However, your insurance company may require a referral for reimbursement.
